CIGNA HealthCare requires all Durable Medical Equipment (DME) be referred to the authorized CIGNA vendor, Apria. Apria Healthcare is America's leading provider of integrated home healthcare products and services.

Please follow these steps when referring a patient to Apria for DME.

1. Complete the Apria Referral Form.

2. Call Apria at (800) 901-3566. Apria's hours of operation are Monday through Friday, 8:00AM - 9:00PM (EST), Saturday 10:00AM - 5:00PM (EST).

3. Apria prefers providers call first then fax the completed form with a copy of the patient's ID card to (800) 723-4288.

4. Make two copies of the completed form. One copy must be kept in the patient health record. The other copy must be given to the patient.

5. Once Apria has processed the DME faxed request, they will ship the DME to the patient directly.
