FEE SCHEDULE

CIGNA GLOBAL FEE $45.00.

What is included in a GLOBAL fee?
The global fee covers ALL routine or standard chiropractic care including spinal and extraspinal manipulations, all standard chiropractic therapies (such as electrical stimulation, mechanical traction, ultrasound, massage, diathermy, manual therapy, neuromuscular reeducation, hot and cold packs and any other therapy), as well as exams and any needed radiographs.

The Mechanics of a “Per Visit Fee”
Claims submitted by CNC Network Providers must include ALL services provided at each visit, together with the appropriate CPT codes for those services. CIGNA HealthCare will process the claims per the terms of the contract and visits for eligible members will be adjudicated using a single CPT code (98940) at a per visit fee of $45.00. Claims will adjudicate at the $45.00 per visit fee (provided the member has coverage on the date of service) regardless of how few or how many services are billed on that date.

CIGNA EOB’s
Regardless of the charges that were billed, the EOB will only show the global $45.00 charge and the associated payment. For all covered services, any deductible, co-pay or co-insurance will be calculated based on the $45.00 charge and will be the responsibility of the member.

Non-Covered Services
Non-covered services, such as maintenance care, may still be provided to the member and you may bill the member directly for these services, provided you first obtain a signed waiver from the member, specific to each non-covered service.

Orthotics / DME
Please note that since CIGNA contracts with national vendors for both DME and orthotics, network providers should not provide either orthotics or DME to CIGNA members. Providers should refer members who need orthotics or DME to these national vendors using the contact information listed below. These services do not fall under the category of “non-covered” services and cannot be billed to the member.
